Referrals to Single Point of Access (SPOA) for Schools with Early Years Provision
Ladywood Outreach Service
Ladywood Outreach Service is part of a provision provided by Ladywood School. It provides a service to all schools in Bolton LA, both primary and secondary schools, in addition to private and voluntary nursery settings (not home based childcare).
Through the Outreach Service, Ladywood are able to share the knowledge and expertise they have developed in order to support partnership settings with the range of pupil learning and communication difficulties now encountered on a regular basis in mainstream settings.
Ladywood Outreach Service has been developed to support the inclusion of pupils with a complexity of learning and communication difficulties, in addition to the full range of autistic spectrum disorders in mainstream schools. Ladywood Outreach Service aims to have a long term impact on inclusive practice in mainstream schools in line with the development of the future role of Special Schools.
A child can be referred to Ladywood Outreach Service once their free funding entitlement commences i.e. the term following their 2nd or 3rd birthday.
The Educational Psychology Service
We deliver an Educational Psychology Service to schools, colleges and early years settings. This involves working with children and young people who have/may have additional needs in a variety of ways, for example, consultations, observations and/or assessments.
We also work with parents/carers, school staff and other agencies to achieve the best outcomes for the child. This may or may not involve writing a report. We provide psychological advice to contribute to Education, Health and Care Needs Assessment.
Service Level Agreement
All school nurseries should request work using the Service Level Agreement for their school or purchase time on an adhoc basis. Schools with 2 year old provision can request work through the same route as the Ofsted Registered Early Years settings for their 2 year old children.
Start Well SEND
Start Well SEND Outreach service is part of a provision provided by Bolton Start Well. It provides a service to support the inclusion of children with special educational needs in Bolton early years provision in school settings.
Bolton Start Well offer this service for children who are:
- Not accessing their full education entitlement
- Have received suspension
- Are at risk of placement breakdown due to severity of need
Examples of support Start Well SEND Outreach can offer are:
- Observe and provide strategies to support children's individual needs
- Support for practitioners to implement the Graduated Approach
- Implement reasonable adjustment audits and adaptations to environments
- Signposting to specialist provisions for further advice and ideas
- Provide written reports
In order to access Start Well SEND, you will need to submit all of the following documents for a successful referral:
- A completed referral SPOA Form - Child Information Sheet - Schools with EY Provision. Please ensure all sections are completed and signed by parent/carer.
- Information that demonstrates how you have already supported the child through the 'Graduated Approach' (Access, Plan, Do, Review process) for at least 2 cycles e.g. IEP's, play plans, provision maps etc.
- A completed EYFS Rating of Concerns Scale.
- An up-to-date Early Help Review that details the concerns and what support has been offered so far, evidence of strategies and documentation gathered. You must also ensure that within information sharing, consent is recorded for SPOA and Start Well SEND.
- If a child doesn't have an Early Help due to social care involvement, you will need to submit a Identification of SEN Form and a Referral form for children receiving a service from social care.

The panel will meet on alternate Tuesdays during term time, and settings will be informed of the outcome by email within a fortnight of panel.
