Transition for Children with SEND

A range of support documents to help you plan for a smooth transition to primary school for children with SEND.

On April 16th, the parents of pre-school children will have learned which primary school has been allocated to their child for September 2021.

This means that you can, and should, now start to put together a plan to make that transition as smooth as possible for all children, but particularly for any child with identified SEND. We have put together a range of things to help you.

If you are working with a child who has SEND, you should;

  • Listen to the online training session on ‘Supporting Transition for Children with SEND'. It will take you less than an hour.
  • Consider the Transition Timeline Document and plan for the things that need to be done.
  • Talk with the child’s parents about their new school and begin to draft up a plan of the ways in which transition will be supported. A template for this with ideas to help you can be found here.
  • Complete the Transition Information Form and share this with the receiving mainstream school by the 21st May 2021. If the child receives Early Years Inclusion Fund, you must also send it to the Start Well SEND inbox at startwellsend@bolton.gov.uk by that date.

If the child is receiving Early Years Inclusion Fund or is on the caseload from Ladywood Outreach, then you will be contacted very soon by either the SW SEND Team or Ladywood to offer you support and more ideas around the process.
